My daughter has ADD and it was suggested from the school counselor that she used a chore chart or schedule to help her keep track of her time. This is what I came up with to help with. It is a blank copy so that parents could add their own chores. It could also be used as a detailed schedule.

The student is able to identify how they are feeling: happy, sad, or mad. The chart prompts them to explain why they are feeling that way. To avoid making copies the chart can be placed in a sheet protector. This way it can be written on and wiped off each week. Documentation can be kept by dating and taking a photograph of the chart.
