Preschool Readiness Rubric
I use this rubric for parent conferences to show progress throughout the school year. I highlight in a different color for each semester so parents can see their child's progress from the beginning of the year until the end. I then add up the total points and then average the class so I can compare and give parent's an idea of where their child is in comparison to their peers.

Autism Social Assessment / Teacher Growth Assessment
This five scaled rubric was created to measure social growth for high school students with Asperger's or ASD. It could also be used for middle school children. I use it through out the year to show growth. It measures seventeen areas in which students tend to struggle. Some of them include hygiene, identify emotions in others and handling frustration, etc. I used this in Illinois as my Type 3 Assessment to show student growth.

Lesson planning grid page for Middle/High School
This is a basic color coded 8 period lesson planning form for teachers to use to plan their weekly lessons in the middle or high school level classroom. The headings are left blank so that the teacher may type or write in the names and times of each period they teach. Writing Assessment Spreadsheet
If your state uses a standard's based assessment rubric for writing, you can enter your students' scores for each strand. These are from Oregon. However, you can change it to fit your state. Once scores are entered (Oregon has 1-6 scale), it averages the whole class' grade at the bottom of the assignment so you can see which strand you need to focus on.
