I use this weekly behavior chart with my primary students. Students color in their happy smiley faces in purple (role model) or green (great day), or the middle neutral smiley face in yellow (warning), or the frown in red (parent contact). Token Board Behavior Management Chart - Zoo Animal Theme
Token boards are used to help a student who is having challenging behaviors.

In this product I have provided you with a chevron themed behavior clip chart with levels as follows:
Purple- Outstanding Day
Blue- Great Day
Green- Ready to Learn/ Good Day
Yellow-Warning

Red- Parent Contact
I have also created monthly behavior logs to put in the students folder to send home what color the student was on each day. On the monthly behavior log it provides the parents with a color code and a number code.

This is a weekly behavior chart for students experiencing behavior difficulties. This chart targets ONE behavior at a time. When describing the targeted behavior, be SPECIFIC. For example, do not use "compliance." Instead use "stay in assigned area." When student is successful (determined by the teacher and support staff), the behavior and goal can be changed if necessary. You can use the boxes as you wish - for checks, smiley faces, and notes.
