Students often have a difficult time, taking good notes and often write down insignificant information or forget to include very relevant information. I hand this template out for each lesson to help guide my students. It is a simple template I created to help structure their notes and keep them on task. It includes, section vocabulary, steps to complete math problems, space for examples, and classwork/homework. It has really helped my students grasp the main points of our math lessons.

I use this sheet to help me stay sane when keeping track of late work assignments. I copy the document onto colored paper and, when I collect homework, if a student doesn't have their homework they fill out the paper and turn it in as if it were the homework assignment. The next day in class, I pull out all of the colored sheet and ask the kids for their work, if they have it, they staple the colored sheet to their assignment. If they don't have their work, I make note of it and file it away.

Pink Slip - for missing homework

This is a strategy I got from Harry Wong (great presenter and author!). It's called a Pink Slip, and students fill it out whenever they do not have their homework. I don't listen to excuses, I just have them fill out the form, and I'll look at it later to see what their excuse is. I keep these on file, and they have come in handy for parent conferences (especially if a student is saying they didn't have time to do their homework due to sports or something or the sort).

Last year I created a "form" for students to fill-out when they did not have their weekly Homework ready to turn in on Friday. This helped me when conferencing with parents. When the child had zeros on a progress report, I could easily take out the HW excuse forms filled in by the student. It did help the students feel more accountable. You'll need to tweek it to your needs. Enjoy!
