CHAMPS tally sheet
Use this to help combine CHAMPS and a color system for behavior.
This spread sheet helps you keep track of CHAMPS rules not being followed. Keep a copy on your clip board throughout the day. As a warning is giving a tally is added to the appropriate letter. After 3 tallies on the same letter there is a color change. No tallies equals purple or whatever color is as good as it gets. !

During this time, students who need to catch up (ketchup) on work will be listed on this sheet. Other students can pick (pickles) an educational activity. This EDITABLE turn in sheet will keep track of everything for you! Use the "notes" section to write down when a student is absent or out of the classroom. When you finally get a chance to grade papers, you will have a record of why a student might be missing their work.
