To be used when a student requests/is assigned a retake of a test or quiz. It forces the student to reflect on how they prepared for this retake and how this time they'll do better. I make students answer these questions by handwriting their answers before I open up/give them their retakes and they are not permitted to retake without this being collected. I have found retake grades to be substantially better than they are without this system.
