Individual Counseling Notes ~ Student Meeting Records
This is a resource for school counselors who meet with students for individual counseling sessions. (Those students who come regularly.) I keep these sheets on a clipboard and grab one when I begin initial meetings with students. Once I start a form for a student it goes into a binder divided by grade level. I can grab the binder and add to my notes as students continue to visit.
