The Spanish version is on the first page, the English version is on the second, and they're exact translations. So even if you aren't a fluent Spanish speaker you can still send one of these home for a parent to fill out. Since most of it is check marks and ages, by comparing the English version and the Spanish version you can look at what the parent checked on the Spanish version and use the English version to see what that check mark means. Since they're one page, they don't require loads of p
