These are 10 different behavior sheets that I use to assess some of my students. I use them for daily sheets or weekly updates. Many of my students also set daily or weekly behavior goals and we use these to assess progress. I left them in Microsoft Word so that you can modify to meet the needs of your students. This resource is a self reflection on student projects and their work ethic. This is a great INTERVENTION for students that are falling behind due to behavioral issues. It can also help you grade students and pinpoint where they are getting sidetracked with their work. It is also an excellent way to deliver feedback on a consistent weekly basis. I have it in a weekly format for a 10 week Marking Period but this is a word document that you can alter to fit your timeframe.
