In-Class Video Permission slip
You can use this freebie to keep track of parental permission for students to watch movies in class that might be outside of their age range. The permission slip explains the academic reasoning and benefit to the movie being shown. There is space to be able to provide the movie rating and the reason.
This permission slip also informs the parents that if they do not comply with their student viewing the film, the student will be provided with an alternate, but relevant assignment.

Very similar to the "Student Survey" that is a free download in my store.
This is a survey I give to my students on the first or second day of the Spanish 2 class. Some questions are in Spanish to start reviewing Spanish 1 material. Other, more complex questions are still in English. (I always tell them "if it's in Spanish, respond in Spanish; if it's in English, you can respond in English").
It helps me get to know them better and I do use their interests to drive some of my lesson plans.

Feedback Trouble - VIPKID? 1 of 7 (Trials)
I am retiring from VIPKID. I have hundreds of lesson feedbacks. Each one is specific to the lesson, and many already have his/her duplicates.
