This is a behavioral plan/contract for students that are repeat offenders in the behavioral department. This doc is designed to help you lay the foundation for a behavioral plan for the student, to increase communication between all staff members and the student, bind the staff members into agreement that this is how the student will be handled and taken care of, and also binds the student to the agreement so they are kept in the loop as to how their behaviors will be handled.
