Daily Folder Page with Rubric
I made some updates with my previous version of this page. The page stays in the students' take home folder and is a great way to communicate daily student performance to families in a short amount of time. I have several incentives that I connect to this page (adult signatures each night, earning mostly 3s or 4s, homework completion, etc). My favorite part about this page is that students are scored on a rubric that includes attention, behavior, citizenship, and academics.

This is a whole school year's worth of a behavior chart. It allows for a code, a parent signature, and a space for the color. This can follow the PBIS behavior management system. At the end of each week, a grade is implemented by the teacher to show to the parent.

Parent Contact Log & To Do List Back to School
Parent Contact Log & To Do List helps me keep track of parent communication through text messages, email, and phone, so I added that type to my log. Also, by request, I added a place to circle whether it is a parent initiated or teacher initiated communication. Depending on your class, you may want to print a log for each student or just one for the class and notate it chronologically. Individualized Positive Behavior Plan
I used this for two of my students to turn their behavior around! They take ownership for their behavior and it is used as a positive, they want to earn 3 SMILEYS!!! :) There is a page for girls and a page for boys.
At the end of the day, they pick a marker and we talk about if they followed directions, listened, and kept their hands to themselves. They get to color in either the smiley, straight or sad! They love this and it has really worked for these students.
