This is an assignment that I give to the PARENTS at the beginning of the school year (or whenever their child enrolls in my class). It is a list of questions for the parent to fill out about their child (learning styles, how I can help their child be the best they can be, etc). It is very informative and is also a great way to get additional contact information too!

Accelerated Reader Marker/Library Card
I create these for my students and customize them with the levels given by the STAR report. I attach the school media center check out card to this card to allow students to easily find AR books on their level. I recreate these each marking period after administering the STAR test. If you print these on colored card stock and laminate them, they last with daily usage. The file is editable and I have it set for my Hollywood theme.
