I use this form for my students to record their Accelerated Reader (A.R.) quizzes on. At the top of the sheet is a place for the teacher to fill out the student's weekly point goal, six weeks point goals and level goal, book levels they are allowed to read, and what types of books they can read. Students are able to determine on their own whether or not they have met their ar goals.

Goal form is an interim tutoring report that is given to administrators, teachers or staff as a measurement of progress. Form is filled out by tutor and parent and signed by both parties as well. Form assists in determination if student should cease tutoring or continue at same or increased hours per week. Form can be completed at predetermined or quarterly intervals.
