This form was created based on common questions I needed answers to quickly. It has saved me from going to the counselor's office to pull the student's cummulative folder every time I had a question. This form is very handy when you're trying to pin-point the reason a student is struggling. If he/she has repeated a grade, he/she should be excelling.
