A self-reflection error analysis checklist to help students monitor their learning. Students complete after each math test and notice mistake patterns. This helps establish plans and goals for future tests. The language of this checklist is age appropriate for 3rd grade and beyond! I use this with my third graders after each test. Just print/copy this checklist and it will be double-sided. I staple it to the top of each math test.

Mistakes on math tests happen for many reasons: A student may not have remembered the necessary steps, didn’t memorize necessary information, didn’t utilize math notes (if allowed to use), didn’t read directions carefully or just made a silly mistake. The philosophy behind Math Repair is to let students have one more look at their math test. A teacher will re-grade the test and attach the note with the new grade.
