This template is similar to my free Cornell notes template, however instead of a field to write down notes there is an area filled with dots that can be used to create graphs or shapes. Like other Cornell notes, it still contains the left-hand column to generate extra thoughts and a summary section on the bottom.

i-Ready Data Wall, Data Notebook Pages, Record Keeping (parts are editable)
I created these fully editable i-Ready record keeping pages to put into my students' data notebooks. I also created the parts needed to make an i-Ready data wall. All the pieces are included, just laminate some poster board and assemble! I simply turned the poster board horizontal. The 4 categories fit exactly down the sides. Test Reflection and Corrections
I use this back-to-back reflection with my middle school math class. At our school, we take monthly cumulative assessments. Students receive a % grade for the whole test, but what does that really mean for their learning?
In the first section there is space for each strand to be listed and the questions associated with that topic. The students then find their score for each topic to identify their strengths and weaknesses. This is especially helpful on cumulative tests for students to pinpoint
