Xtra Math Data Collection Form

A monthly chart that students can use to self-monitor their scores each time they complete Xtra Math. This is great for allowing students to set goals and reflect on their progress throughout the year. It also is great data to show parents. I use this in my students data binders. Each day when they come into class, they complete Xtra Math. At the end of "morning work" or at the end of class, I check their scores and they record them in their notebooks.
