Guided Math Centers Reflection Sheets - Inspired by Dr. Nicki Newton

In her excellent book, Guided Math in Action, Dr. Nicki Newton describes her use of reflection sheets for math centers as a means of providing accountability to her students for the work they are to complete in math centers. Being inspired by the information in the book, I have created these three sheets. The sheets increase in difficulty as they progress. You can use the first one to introduce the activity and build up the skill, then use the other forms as the year progresses. They can als

Beginning of the Year Mathematics Survey - Includes Spanish translation

I have used this beginning of the year survey for my math classes over the past couple years. It's a great way to get to know students! I keep it handy for reference throughout the year! I usually copy this back-to-back with a separate document that contains math questions to serve as a formative assessment tool for our first unit. This way students can take their time filling out the survey in class and continue on to the math problems on the back if they finish before their classmates.

Test Correction Forms

After every test, I require students to fix all of their mistakes (regardless of how well they did). This form requires students to not only find a new answer, but to identify where they went wrong, so they hopefully won't make similar mistakes in the future. I allow students to get help and use any resources they can for these corrections, and they count as a quiz grade. This also helps students improve their grade a bit if they didn't do well on the original assessment. This works very well
