Function Tables (Input/Output Tables)

Set the Rule and then complete the Input/ Output Table. It can be used for addition, subtraction, multiplication, or division. This can be used with Input/ Output task cards or you can partly fill in the form your self to have the students figure out the rule and the missing numbers. Set a rule, and have the students complete the table with their choice of numbers. There are so many ways it can be adapted to meet the needs of your students. Enjoy!

School Function Assessment Grid

This is one grid that could be used to report the sections of the SFA that your team used. Simply delete what is not needed. Only complete the column of criterion cut-off scores that are appropriate for the grade level of your student. *You will still need the scoring manual and your score sheet.* The graph on the back of the recording form is nice to present a summary of the information during the IEP, parts of this summary chart might be used in your PT report only.

Basic Parent Questionnaire for Meet and Greet or Orientation - Editable

Created by

This is a single-page, simple parent questionnaire that I leave on tables during our meet and greet the day before school starts. There are 3 questions: one about dismissal routines (for first day and also for rest of year), one regarding food allergies/restrictions, and one asking for parent info to share in a class-only directory. It's a Word document with basic fonts so you can edit it to best fit your needs.

Basic ABC Circle Sheet

This is a generic ABC data sheet. There are 6 columns: date, start time, antecedent, behavior, consequence, and end time. The antecedent, behavior and consequence columns have examples in them so staff can just circle which one occurred. The examples are things that most often occur but there is a space for staff to write their own answer. Having a circle data sheet leads to more accurate data because staff are more willing to fill it out. This data will be useful for determining a potent

Free Editable Checklist

Free editable checklist. This is a basic checklist that allows you to mark off students for excursions, students who have completed activities etc. I have used this for Kindergarten activity tables so that I could quickly mark off names of those who had completed the activity. I have also used these in high school attached to submission boxes so that I had a quick, easy record of who had handed their work in.

Back to School Night Wish List Ticket

With schools providing less and less money for basic classroom supplies, it's only fitting that teachers dish out their own money to get things done. (Of course!) That's when donations come into play. I have used these little wish tickets for the last 4 years, and they've provided many happy returns. I set these out on the tables at Back to School Night for parents to take if they's like to donate. There are inexpensive items like facial tissues and paper towels (available at Dollar Stores) and

Editable so the goals can be changed to suit your school/term/year level context and the multiplication tables could be changed for other number facts, eg. addition, subtraction, or division facts. I used this in student portfolios to show progress over the term to parents at interviews.

This simulation is designed to help understand how the primitive economy functioned before the emergence of first agrarian civilizations. The prime goal is to develop historical empathy and feeling that history saw the economic practices that were modified but remained unchanged at their core for ages. The competences developed due to the simulation are as follows: • basic general knowledge deepened due to the economic activity, • problem solving and decision making, • interaction (work in a

PLS-5 Template | Speech Therapy Assessment

For my first ever product (!) I decided to offer my Preschool Language Scales - Fifth Edition (PLS-5) template. It includes a test summary, score table, and summary sections. It's available as a Word document and formatted so you can quickly and easily copy and edit it in your own report. With my templates I try and solve two problems: a) writing and formatting the basics of a test can take up unnecessary time, leaving clinicians less time to write about what the results really mean; b) parents

Basic Skills Checklist for Kinder

I use this checklist to check off what students know or don't know yet. Using pencil allows me to erase and make changes as the year progresses. I can also change skills, or words, each year if necessary, and I can use this as a goal-setting tool with each student. I provide a copy to parents at the end of each 9 weeks so that they know where their child is and where he/she is headed since most of this detailed information is not part of our report card. Weaknesses can be seen at a quick glan

I keep these sheets for each child, in addition to my reading comprehension skills sheets, that has a goal in this area. It makes it easy when I need to average scores for progress.

Super basic articulation rubric for kids to monitor each other and themselves when speaking. I usually do this a few times a year. Each student fills out a form on each speaker and one about themselves. These print 2 per page and could easily be laminated and reused. (Sample activities would be having the student report on menus from restaurants, tell how to do something, retell a story, or describe a picture scene.)
