This is a form I use to monitor student behavior in 15 minute intervals when I go do an observation for a student that has Attention Deficit disorder related behaviors. This recording sheet allows students to record their grades for homework, classwork, quizzes, tests, projects, and book reports. I have the percentages for each of those types of assignments, but the file is in Word, so the teacher can change those percentages to meet the needs of his or her classroom. My students update this sheet when they receive their tests, projects, etc back, so they can keep track of their progress in my class.
