This document, while not FORMAL to schools, has helped me document a students' behavior quickly. I can place it on a clip board and walk around with it throughout the day. It's especially practical when having a parent-teacher conference or a meeting with the counselor or principal. It gives a great view of how many times a student was spoken to, redirected, and if they continued the behavior or not. Hope you find it useful!
