***THUMBNAILS show error in file, but everything is aligned in the docx file!*** Use this form to leave notes for parents about the students' speech therapy sessions. Cut the form in half to send home with the student (or leave at the day care / preschool). Or log your notes on the computer then email or print for parents. Tutoring Lesson Parent Note Template
This is a template that you can use for sending home notes to your tutoring kids. All you need to do is print it out, or fill it out on the computer and then send it home at the end of each of your tutoring sessions. This helps the parent/guardian know what the student learned during their time. If you take a picture or save the document you can also remember what you did during the tutoring session.
