Even though it was a great idea for teachers to see each standard and how it applies to each grade level, I find that when it comes to lesson planning, I wanted to see only standards that apply to my grade level. The state does not have this format available, I decided to make my own. I hope this helps my fellow ELA teachers in their daily/weekly planning!

This is an EXTREMELY simple form I keep in my binders for each class. There is a copy of this form for each student. I keep a clipboard of labels nearby during class, and when I note a behavior that I want to record, or if there is an incident in class, I record it on a label. Later when I have time, I stick the label on the observation form. Now my documentation is a piece of cake. If I need to talk to parents or administrators about a student's behavior in class, I just have to look at my log.
