This is a newer form I have created for my students to record their reading information on. The top of the page provides their reading goal information. Also, there is a place to write the book levels for fiction and non-fiction books. Also, I require my students to retest under my name on quizzes they do not pass. There is a column especially for this. By using this form, students will be able to know at all times whether or not they are making their reading goal or not.
