For each book students record: Date they tested, Book Title, Book Level, Percent Correct on test, and number of Points Earned.
This Cheat Sheet was made for students to focus on the basic fraction to percent, to decimal conversions. I would have the students memorize it andquiz them on it the following week.

This is a really fun idea for any stage as you can edit the questions to increase difficulty or decrease.
