I created this tracker for students exhibiting challenging behaviors. This tracking system was used in conjunction with positive reinforcers/incentives that appealed to the student. For example, if the student achieved a score of 100 then he or she would gain access to a preferred incentive. This incentive was usually dependent upon the function of the students behavior (i.e. escape, attention, attainment).
