15 minute Behavior Observation Feedback Form

This is a form I use to monitor student behavior in 15 minute intervals when I go do an observation for a student that has Attention Deficit disorder related behaviors. The form can be modified to check for different behaviors. After I monitor the student I typically pull out the student to my office and discuss what I observed using positive reinforcement and sometimes tangible rewards to encourage correct behaviors. The goal is to increase the amount of time and see a corresponding decrease

I use this behavior contract with my students who need just a little bit more motivation. It is very effective if used consistently and with parent involvement. I usually have the students work towards a personal goal/reward to motivate them to follow the tasks listed on their contract (i.e.lunch with teacher, extra iPad time, etc.) I truly hope this product works for the needs of your class! ** If you do not have the fonts downloaded on your computer, you can download them from Hello Fonts
