Therapy Notes for Parents - Session Summary
***THUMBNAILS show error in file, but everything is aligned in the docx file!*** Use this form to leave notes for parents about the students' speech therapy sessions. Cut the form in half to send home with the student (or leave at the day care / preschool). Or log your notes on the computer then email or print for parents. The form is in DOCX format so you can edit as needed. Fill out the "Today, we worked on..." section with brief descriptions of the student's goals (e.g., making the /s/ sound)
