Communication and Documentation

Keep parents in the loop by having staff or students (depending on the age) fill out this daily log of what happened. This is a great way to get parents involved! It also works really well for documenting individual student progress. Also, by documenting the child's daily activities you can begin to determine patterns, struggle, and strength areas for each student.

Having Students Take Initiative for their Grades

Here and there I have my students fill one of these out after looking at their grades, so that they can become more responsible. The great thing about this is it allows them and their parents to see what actions they can take in order to get a better grade. Something I really believe is that most students who have bad grades, have that grade because they don't know how to get a better grade. This sheet allows students to own up to why they have a bad grade, while also allowing them to ask themse

Parent Questionnaire for Back to School in English and Spanish

I have used this questionnaire for the past ten years and had it translated into Spanish as that is the primary language of most of my students. This really helps to delve into the interests of the students, which is especially helpful to use as an incentive (i.e. if they like soccer I add soccer themed items to the treasure chest) and conversation starter. Just this year a parent hadn't remembered to add that her child has an allergy to honey until she saw this form. Enjoy and welcome back to s
