I have used this sign in form in addition to whatever the school offers so I can have the information for grading and CUM's. I added the section for a t-shirt since we try to give shirts as incentives whenever possible, but feel free to "tweak" it as you see fit.

Kindergarten Behavior System
This is the behavior system I used when I taught kindergarten to reinforce good behavior. I sent this document home to parent in the beginning of the year and explained how I use sticker charts and the alphabet to implement good behavior in the classroom. Feel free to use these ideas at home as well as in the classroom! This document is also editable, so you can add your own ideas to it if you like! Hope this helps!
