We created this resource to increase parent and student involvement by establishing expectations and goals. We chose to focus on Reading and Math but you can use these for other subjects as well. There are 2 commitment forms; 1 for the parent/s and 1 for the student. We would suggest meeting with each parent, whether it be face-to-face or virtually, in order for this to be successful.
