As the behavior RtI team leader, I am often asked to come in an observe a student. Often times, I utilize this form first to assist the teacher in his/her baseline of positive/negative interactions with the student. I truely believe in the motto "What we pay attention to, is what we will continue to get!"; therefore, we need to understand what kind of feedback we are giving the student. This form is very helpful in obtaining the ratio data and assisting in a discussion of what could be a quic
