I use this with my middle school students and English Learners after weekly or bi-weekly quizzes so that they can reflect on how they did on their quiz. This goes in our data folders after each quiz or test so that they have a moment to reflect on any mistakes they make and how to grow from them.

This can be used each month for Fact Fluency progress. After students take their monthly fact fluency quiz, they can fill in how many they got correct. This is a great way for students to track their own progress throughout the year! Includes both Addition and Subtraction, but can be changed to include Multiplication and Division. Months September-June, but can be changed. Number of problems correct= 60, but can be changed.
