Use this weekly Log to communicate with parents how their child's morning and afternoon were. What they need to continue working on in regards to behavior and any extra comments from teacher at the bottom. This could be for specif events/behaviors that happened during the day or for a positive and happy note home about the day. Parents initial at the bottom of the paper that they say the chart and comments each night and it comes back to school the next day.

Great Reflection sheet for upper elementary/middle school students. It starts by asking what happened, why did you do it (emotions), what was I trying to communicate, and consequences of choices. Everything can be check marked or circles so minimal writing is involved. At the end it asks what can I do differently next time?

I graduated with my Bachelor's degree in Special Education in 2016 from Northern Illinois University. I am currently in a Graduate Program through Northern Illinois University to become a BCBA.
