This Functional Behavior Assessment form is used to interview caregivers of children in order to collect indirect data to determine functions of maladaptive behaviors. The Indirect FBA: Parent Interview provides questions in order to get a clearer picture of behaviors present at home, school, etc. This resource includes a Spanish and English version of the Parent Interview in order to facilitate language differences.
