We all know that collecting makeup work can be a disaster! To make that necessary evil easier, I created this form to make it less of a hassle. Each year I give a reading survey to my students to find out each student's reading confidence, likes, dislikes, favorite genres, favorite authors, etc. Not only will this survey give you a better feel of your classes reading attitude it will help you create a love of reading in your students. This is a great tool to have at the beginning of the year to help guide your readers to books geared towards their interests. You can keep these in a folder and reference back to it as the year progresses.
