I use this to keep track of my students' goals each week. I meet with each student and we decide on a goal to work on. Sometimes we continue goals from previous weeks. I list their names, their individual goals, and then my comments on progress or other important information. Class goals can range from behavioral to academic. I have 10 slots on the page but more sheets can be printed out for larger classes.
