Parent and Family Input for IEP Meetings

Send this parent/guardian survey home before IEP meetings to increase family participation. Meetings can make families feel like they're "on the spot" or under pressure, and this survey takes the pressure off. They will have time to think about their answers, and will feel like part of the team. It will also give the special education team information ahead of time so all parent questions and concerns can be addressed during the meeting. It's a win-win! Have students sign in and out when they leave the classroom. This will help you notice any patters in students leaving (are the leaving for a long time? during the same subject each day?), as well as know where they are in case of an emergency. There are check boxes for bathroom and water, as well as a fill in the blank space for other. Print a bunch to leave on a clipboard by your classroom door.
