This is a document to record fluency passage data. It's simple and straight-forward. Although you don't, necessarily, need all the information to calculate WPM, I like to have it so I can see what, exactly, the reader is struggling with while reading. That is why there are spaces for omitted, inserted, and incorrect words. There are 2 pages of the same document for 2-sided printing. I have been able to use this document a great deal. This data sheet can be used to record up to 20 high frequency words you are working on at one time. I use it to test the words for a week and record the student's overall score. If they are still working on a word, I just write it over into the next week! I hope this is helpful for you! :) There are 2 of the same sheets so it makes it easy for 2 sided printing!
Here is a document that I use in an IEP notebook. It's a quick overview of a student's IEP and basic information. It has been helpful, for me, to have all of this information in one spot so I don't have to dig for information!
