This tutoring report can be used with students that you tutor individually or in small groups. It serves as good documentation and a form of accountability for yourself, parents, and other teachers/support staff who are helping a student. I modified this from a couple of different student self-assessment forms I found online. This is perfect for intermediate students to reflect on their classroom behaviors as well as how they feel about themselves as a student. The rating scale matches the grading scale of our school which ranges from not proficient to advanced. You can edit this document though if you want the statements or rating scale to reflect something different.
