This form is to be used by a Speech Language Pathologist as an observational rating form in the classroom setting, either special education or regular education. It is intended to give the SLP information of how a child is functioning in the classroom setting.
These documents allow you to observe a student in his/her environment and take note of the classroom environment and the way in which it may impact the student's auditory learning. This can be used for observation forms for a FM system and used as trial data before an FM is put into place and during the trial use of the FM system.
