This is to be used for checking the MLA formatting for a Works Cited as part of a research paper. This was a 25 point assignment. I usually put this on colored paper and cut it into four squares and have either the student grade themselves or have the teacher grade it and attach. These checklists would also be helpful in a computer course regarding formatting or papers.

Chicago Public Schools Parent Transition Survey in Spanish for Special Education. This form is similar to the form available on SSM for Special Education teachers and Case Managers. Feel free to download this form and send it to parents to fill out. Google translate could help if the parent types or writes the response in Spanish. This should help gear transition goals for students regarding life skills, employment and education goals.
