This table is to help record a child's behavior throughout a period of time. It allows the educator to write down a specific behavior, time and date plus possible reasoning for said behavior. In the bottom I have included examples of behavior that might be of interest to be recorded. This tool is also excellent example for a parent in order to help establish a record for behavior specialist and other professionals working with the child.

This is a document used for Substitues teachers coming to a unkown classroom. It can be edited to help the substitue understand students needs, wants and routines. It is great for important daily important information such as pick up, drop of time and nap time routine as well as potty training status and child allergies.
