These are a MUST HAVE for MAP testing! These goal sheets are for students so they know what their goals are for MAP testing. There are three sheets for each test: Fall to Winter goals, Winter to Spring goal, and Fall to Spring goals. These are the individual cards that can be used for each test. There are also three pages included that have all three tests on one page if you would like for each student to have a page with all of their scores together. Included are Reading, Language, and Math MA
These are used to highlight students' independent Lexile levels. Highlight the numbers and put colored dot labels in the circles to help students to know what color they're looking for in the library. File includes 4 different pages of lexile cards, 3 cards per page. Levels on cards are: BR-700, 200-900, 300-1100, 500-1300.

This chart is used to test students "words per minute" if they are reading below grade level. Give them a passage and allow one minute for students to read as far as they can. Total words per minute read - errors = words per minute.
This behavior plan was made to break up the day into sections. Fill in the boxes with the subjects and special areas for each week. This way your student is only focused on a short part of the day instead of the entire day as a whole.
Teachers always have so much data! It’s hard to remember how each student did on everything. So I created this chart to help me remember how students did on the DIBELS test. Each passage has a box for WPM, accuracy, and retell. The comment section I use for anything that catches my attention and I want to remember. I hope this chart helps you as much as it helps me!

Give students a "Question of the day" and cut these forms for them to write their answers. They turn it in and the teacher chooses one student with the correct answer for a small prize. See my Class Bucks... that would be a good reward! This is good for a quick assessment to see if students understand a certain topic or skill.
