This chart is used to test students "words per minute" if they are reading below grade level. Give them a passage and allow one minute for students to read as far as they can. Total words per minute read - errors = words per minute.
This behavior plan was made to break up the day into sections. Fill in the boxes with the subjects and special areas for each week. This way your student is only focused on a short part of the day instead of the entire day as a whole.
Teachers always have so much data! It’s hard to remember how each student did on everything. So I created this chart to help me remember how students did on the DIBELS test. Each passage has a box for WPM, accuracy, and retell. The comment section I use for anything that catches my attention and I want to remember. I hope this chart helps you as much as it helps me!
Use this log to keep up with communication between teacher and parent throughout the year. Documentation is key! Zipped file also includes an All About Your Child form for parents to fill out so you can have more information on their child. Terms of use included.
Use this volunteer form to get an idea of which parents are interested in helping you during the year. Also find out exactly what they would like to help with!
Use this at Open House to keep a list of students and parents that came to meet you! There is space for 25 students/parents to sign-in. You will also get feedback of parent goals for their children.
