I use these RTI data sheets each year in my classroom for any student who has an RTI. I place these in a binder tabbed by student name in order to keep all of the information and data organized for each child. I then transfer the data to our school's RTI form at review time.
This is a tool I created to document ELA standards for all my preschoolers in Ohio. Each student has a tab in a binder with all of the pages. Fall is completed in blue and Spring is completed in red. We circle their score in the corresponding color and then write the evidence on the lines. This makes it easy to pull documentation for progress reports and ETR's as well!
During such a hectic time when parents have had to step up to teach at home, I thought it would be fun to give them an award! I will copy this on colored paper or card stock.
This is one document that can be printed front to back to hold essential information for all students in one place. No more flipping through binders! Headings appear broken in the preview, but are not once opened. :)
