When we initiated the use of emergency Epinephrine in our school district, I had to research and write an order for our school. This form can be printed and used with no editing required. Just write in the school district name at the top and take it to the supervising physician for review and signature. Then, this form can be taken to the policy writers for integration into school policy.

I created this form to keep track of Epi-pen maintenance and use at our schools. We are required to do checks on them AT LEAST monthly if not more. I just write the dates in the boxes. I like that it's all on one easy to use form. I hope this helps and maybe saves you some time!
