This is a form that I use for teachers that work with the students that I support. It gives basic information which I attach to a physical copy of the IEP When I had them to teachers for their reference during the school year. When an IEP changes I will make changes to this document and hand the teachers a NEW fresh copy of the IEP along with this file to update and highlight any changes to the IEP.
