This is great for a quick check in with your students to see how they're doing. I leave this by my desk and my students only fill it out when they want to speak with me but don't feel comfortable or brave enough to come up to me in class to talk about something they're struggling with. Each sheet has a different title to select based on what you want to call these slips in your classroom. You can use this how you see fit in your classroom. Enjoy!
